Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.
- Co-morbidities and past medical history including tetanus vaccination status - History of injury including significant water, animal involvement or environmental exposure - Details of all treatments offered, and efficacy to date e.g. type of dressings used, antibiotic history
