Cat 1 · 30 days

appointment within 30 calendar days. - Upper GI symptoms with anaemia, weight loss, epigastric pain - Dysphagia - Para-oesophageal hernia with symptoms suggesting recurrent obstruction - Abnormal imaging results suggesting oesophageal gastric pathology

Cat 2 · 90 days

appointment within 90 calendar days. - Severe volume reflux with episodes suggestive of aspiration and/or severe impairment to quality of life - Para-oesophageal hernia - Barrett’s oesophagus with low grade dysplasia

Cat 3 · 365 days

appointment within 365 calendar days. - GORD resistant to medical therapy - Barrett’s oesophagus without evidence of dysplasia

Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- History of the condition including previous treatments tried and efficacy - Patient's co-morbidities - Medications - E/LFT - FBC - Iron studies - Any abnormal imaging reports and/or endoscopy reports If a specific test result cannot be obtained due to access, financial, religious, cultural or consent reasons a clinical override may be requested. This reason must be clearly articulated in the body of the referral.

Out of scope / wrong door

The following conditions are out of scope for this service: - Aesthetic or cosmetic surgery - Abdominal lipectomy - Breast reduction / augmentation - Appearance medicine - Vasectomy (unless within scope of the local health service) - Reversal of vasectomy
