Cat 1 · 30 days

appointment within 30 calendar days. - Peri-stomal complications - Issues with ongoing leakage - Pressure injuries and/or wounds resulting from underlying hernia - Skin reaction to appliance - Suspected Pyoderma Gangrenosum

Cat 2 · 90 days

appointment within 90 calendar days. - No category 2 criteria

Cat 3 · 365 days

appointment within 365 calendar days. - No category 3 criteria

Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- Co-morbidities and past medical history - Relevant clinical history including surgical history (date/place of surgery) and stoma type (Ileostomy Urostomy Colostomy and other) - Details of all treatments offered, and efficacy to date e.g. date of commencement of any antibiotics with dose prescribed
