Cat 1 · 30 days
appointment within 30 calendar days. - Non healing pressure injury Stage 2 despite pressure relief/offloading for 2-3 weeks - Stage 3 and 4 pressure injuries - Unstageable pressure injuries - Suspected deep tissue injuries - Medical device-related pressure injuries
Cat 2 · 90 days
appointment within 90 calendar days. - No category 2 criteria
Cat 3 · 365 days
appointment within 365 calendar days. - No category 3 criteria
Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.
- Relevant wound history (e.g. duration, description, and size, wound initiating event and any previous surgical debridement or reconstrution) - Co-morbidities and past medical history including disabilities - Details of all treatments offered, and efficacy to date e.g. pressure relieving interventions, type of dressings used, antibiotic prescribing history
