CPC windows

Cat 1 · 30 days

appointment within 30 calendar days. Medically stable patients who are: - Immediately post discharge from the Gold Coast HHS who require short term assistance within the community to prevent hospital re-admission.

Cat 2 · 90 days

appointment within 90 calendar days. - No category 2 criteria

Cat 3 · 365 days

appointment within 365 calendar days. - No category 3 criteria

Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- All referrals need to be discussed with the MAPS team leader, please call 1300 00 4242 (1) - Patient/decision maker agreeable to referral - Patient has a nominated GP - Other health providers involved in care have been notified - Home Visiting Risk Assessment completed

Out of scope / wrong door

The following conditions are out of scope for this service: - Paediatric patients – under the age of 18 - Patients from Residential Aged Care Facilities (RACFs) - Patients with high level care needs - Patients with high-risk Home Visiting Risk Assessments
