appointment within 90 calendar days. - New onset Polymyalgia Rheumatica with typical shoulder/hip girdle features with raised ESR/CRP
- Known and treated PMR established on steroids requiring further escalation of management

appointment within 365 calendar days. - No category 3 criteria

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.
- Relevant history of onset, recurrence, acuity (muscle pain, morning stiffness, headaches, amaurosis fugax)
- Details of treatments offered including glucocorticoids initial dose, tapering and current dose and/or DMARDS (if available)
- FBC, E/LFT, ESR and CRP
