Essential referral information
Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.
- History (if relevant) including: - claudication distance - rest pain - ischaemic changes - any antiplatelet medication regime - peripheral pulses: femoral/popliteal/foot - Risk factors particularly smoking and diabetes (if relevant) - Recent cardiac tests, including stress test results - Duplex USS scan results - U&E - FBC & coags - BSL - Lipid profile
Out of scope / wrong door
The following conditions are out of scope for this service: - Asymptomatic / cosmetic varicose veins (unless within scope of your local health service) - Lymphoedema without arterial or venous disease
