Cat 1 · 30 days

appointment within 30 calendar days. At risk of hospital admission or nasogastric tube insertion Concerns with >3 of the following: - Faltering growth - Less than expected weight gains - Decline in growth trend - Reduced oral feeding - Oral/Feeding Aversion - Suck feeding concerns - Delayed oral motor skills impacting on ability to eat age-appropriate foods or drink Swallow safety concerns (e.g. recurrent chest infections, coughing, wet breathing) Pressure feeding and related difficulties with reading infant cues (e.g. over or under responsive cues, unpredictable state transitions, co-regulation) Significant parent stress/worry-in relation to feeding

Cat 2 · 90 days

appointment within 90 calendar. - Not at immediate risk of hospital admission or nasogastric insertion - Enteral or oral feeding where condition has the potential to require more complex or emergent care if MDT assessment is delayed. - Requires MDT tube weaning assessment as showing signs of readiness to tube wean. - Condition is unlikely to require more complex care if MDT assessment is delayed.

Cat 3 · 365 days

appointment within 365 calendar days. - No category 3 criteria

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- Contact details for parents/carers - Reason for referral - Medical history - Feeding history

Out of scope / wrong door

- Resides outside of GCHHS catchment - Not requiring MDT support for feeding - Eligible for NDIS therapies or accessing private support and not showing signs of readiness for tube wean. - Patients eligible for CDS mealtime clinic.
