appointment within 30 calendar days. - Malnutrition/ Faltering growth/ Failure to thrive (all ages) - Iron and other micronutrient deficiency (< 24 months of age) - New diagnosis of food allergy, where growth is faltering or substitution of food is required (e.g. use of hypoallergenic formula for CMPI/ coeliac disease) - Eating Disorders

Cat 2 · 90 days

appointment within 90 calendar days. - Suspected or confirmed food allergy/ intolerance and child is thriving (all ages) - Feeding issues/fussy eating, without any complications (all ages) - Micronutrient deficiencies (e.g. Iron deficiency) (>24 months of age) - Constipation with encopresis - Bowel disorders (e.g. chronic constipation, encopresis, IBD, chronic IBS, bowel surgery). - Weight management (all ages) for BMI>85th centile, general healthy eating advice. - Dyslipidaemia, HTN

appointment within 365 calendar days. - No category 3 criteria

Out of scope / wrong door

- Pt does not identify as either Aboriginal or Torres Strait Islander - Patient accessing other GCHHS specialised dietetic community or outpatient service - Patient accessing dietetic services through a non-government organisation - Patient accessing or are eligible for dietetic services through an approved NDIS package - Patients residing in a residential aged care facility (RACF) - WorkCover patients and DVA Gold Card holders - From 1 November, 2025 - Patients accessing Levels 4 to 8 - Support @ Home Programme package through My Aged Care (MAC) - Prior to 1 November, 2025- Patients accessing Level 3 or 4 Home Care Package through My Aged Care (MAC) - Do not provide services in conjunction with GP Management Plan or Team Care Arrangement
