Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- Relevant medical/surgical history including: - Antenatal or perinatal factors such as drug/alcohol exposure, pre-term delivery or low birth weight - Recent investigations including audiometry and vision test reports - Relevant developmental concerns and any current services involved - Relevant social history including: - Parental relationship issues - Mental illness - Substance abuse or disability - Family in crisis - Child Protection or Department of Children, Youth Justice and Multicultural Affairs involvement [https://www.cyjma.qld.gov.au/child-family/protecting-children] - Literacy/language considerations for parent/carer

Out of scope / wrong door

The following conditions are out of scope for this service: - Health and developmental assessment for children and young people who are NOT involved with the Department of Children, Youth Justice and Multicultural Affairs. [https://www.cyjma.qld.gov.au/protecting-children] - Educational assessment of children/ young people in out of home care - Allied health, counselling or psychology interventional/services for children and young person or carer - Forensic or sexual abuse evaluation of any person 14 years and over - Forensic evaluation for young people presenting with assaults not considered due to abuse
