Cat 1 · 30 days

appointment within 30 calendar days. - Have complex needs that require active management to support engagement, access, service optimisation and wellbeing - Have insufficient supports to sustain current need in the community - At imminent risk of non-acute hospital utilisation

Cat 2 · 90 days

appointment within 90 calendar days. - No category 2 criteria

Cat 3 · 365 days

appointment within 365 calendar days. - No catgeory 3 criteria

Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- Patient/decision maker agreeable to referral - Patient has a nominated GP

Out of scope / wrong door

The following conditions are out of scope for this service: - A person requiring a diagnosis - A private patient or patient previously unknown to GCHHS - An inpatient being managed by a treating MDT; or a community member with sufficient supports and service in place. - Already known to another nurse navigation or specialist case management service - A person living in a residential aged care facility (RACF) - Maternity patients - Paediatric patients - Presentations that would be more appropriately assessed and managed by specialist mental health services e.g. where a primary psychiatric diagnosis is the most likely cause of the patient's presenting symptoms - Outside Gold Coast Catchment - *If the patient lives outside the Gold Coast Health catchment area and you wish to refer them to one of our services, please indicate relevant medical or social information that will assist with the processing of your referral
