Cat 1 · 30 days
appointment within 30 calendar days. All hypertension referrals would be triaged to General Medicine, unless the referral contains specific clinical indicators warranting sub-specialist review. Referrals may be allocated to Kidney Medicine, Cardiology, or Endocrinology where clearly justified by comorbidities, diagnostic complexity, or suspected secondary causes of hypertension. - Confirmed or suspected phaeochromocytoma
- Confirmed or suspected Cushing's Syndrome
- Confirmed or suspected Primary Hyperaldosteronism with potassium <3mmol/L
Cat 2 · 90 days
appointment within 90 calendar days. - Primary hyperaldosteronism (Conn's syndrome) with potassium ≥3 mmol/L
- Patients suspected of having any other secondary endocrine cause for hypertension
Cat 3 · 365 days
appointment within 365 calendar days. - No category 3 criteria
Essential referral information
Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.
- Medical History & Clinical Background:
- Detailed medical history emphasising any cardiac, renal, or endocrine concerns.
- History of blood pressure (BP) measurements (preferred both arms), including 24-hour or home measurements if available.
- Current medication list, including over the counter (OTC) medications, and detailing all treatments tried, including efficacy.
- Endocrine Investigations:
- 1mg dexamethasone suppression test and/or 24-hour urinary free cortisol levels.
- Plasma free metanephrine and normetanephrine levels.
- Imaging & Other Diagnostic Tests:
- Chest X-ray report.
- Laboratory Investigations:
- Full Blood Count (FBC).
- Electrolytes, Liver Function Tests (LFTs).
Out of scope / wrong door
The following conditions are out of scope for this service:
- Pre-diabetes
- Stable, well-controlled type 2 diabetes
- Newly diagnosed type 2 diabetes and not acutely unwell
- Referrals where the primary problem requiring attention is not directly related to the diabetes and should be directed to another speciality service e.g. chest pain for investigation should go to cardiology
- Dietary advice for weight reduction, high cholesterol, hypertension or CVD in patients with diabetes
- Newly diagnosed primary hypothyroidism, including subclinical hypothyroidism – Note: in women of child bearing age who are pregnant or wishing to become pregnant or not using contraception, thyroxine should be commenced and titrated, aiming for a TSH less than 2.5
- Positive thyroid antibodies with normal thyroid function
- Osteopaenia
- Routine uncomplicated osteoporosis
