Cat 1 · 30 days

appointment within 30 calendar days. - For optimum care, these conditions should be seen within 2 weeks: - New NG/NJ feeding/ scripts for supplements or feeds - New Exclusive Enteral Nutrition/Polymeric Diet (for inflammatory bowel disease) - For optimum care, these conditions should be seen within 4 weeks: - High risk moderate-severe Malnutrition - HEN patients with acute issues - Stable NG/NJ feeding

Cat 2 · 90 days

appointment within 90 calendar days. - Ongoing scripts for oral nutrition supplements

Cat 3 · 365 days

appointment within 365 calendar days. - No category 3 criteria

Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- Reason for referral - Relevant Medical history - Treatment to date including reason for commencement on home enteral nutrition - Type of enteral nutrition prescribed - Any relevant discharge arrangements If a specific test result is unable to be obtained due to access, financial, religious, cultural or consent reasons a Clinical Override may be requested. This reason must be clearly articulated in the body of the referral.

Out of scope / wrong door

The following conditions are out of scope for this service: - Patient requiring gastrostomy or parental nutrition - Patient accessing other GCHHS specialised dietetic community or outpatient service - Patient accessing dietetic services through a non-government organisation - Patient accessing services through an NDIS package - Residents of RACF or patients requiring home visits - Patients requiring advice on weight management, healthy eating and other conditions not requiring enteral nutrition support
