Cat 1 · 30 days

appointment within 30 calendar days. - Microscopic haematuria with a rapid decline in kidney function (>25% decline in eGFR in 6-12 weeks) and a urological cause is considered unlikely

Cat 2 · 90 days

appointment within 90 calendar days. - Persistent microscopic haematuria with coexisting proteinuria and stable or slowly progressing decline in kidney function (ie <25% decline in eGFR in 6-12 weeks) and a urological cause is considered unlikely - Previously diagnosed chronic glomerulonephritis who require ongoing specialist follow up

Cat 3 · 365 days

appointment within 365 calendar days. - Asymptomatic persistent microscopic haematuria where a urological cause is considered unlikely

Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- Presence of comorbid conditions such as hypertension, diabetes or vascular disease - List of medications - Recent BP results - Presence or absence of pain - ELFT and FBC results - Serial urea, creatinine and eGFR results - Urine albumin creatinine ratio (ACR) or urine protein creatinine ratio (PCR) (ideally early morning sample but a random sample is acceptable) - Urine midstream M/C/S (including testing for red cell morphology, casts and crystals preferable) - Ultrasound (kidney, ureters & bladder) or CT IVP (intravenous pyelogram) results
