- Indigenous Status information eg. Aboriginal, Torres Strait Islander, Both Aboriginal & Torres Strait Islander - History of chronic condition: eg. New or chronic condition to continue >6 months - Identification of chronic condition/s - Goal of referral including priority concerns - Significant medical or social history
Out of scope / wrong door
- Assigned home care package, level 4 - No fixed address and no GP details - If primary diagnosis is mental health and/or substance related psychosis - Mental Health and Alcohol and other Drugs represent majority of the Emergency Care - Already known to another nurse navigation or specialist case management service - Residents of an aged care facility (RACF) - Patients who do not identify as Aboriginal and/or Torres Strait Islander - Patients who are experiencing diagnosed cognitive impairment - <18 years of age
