appointment within 90 calendar days. - Suspected Ankylosing Spondylitis/ Axial Spondyloarthritis with or without peripheral symptoms and/or extra articular manifestations - Known Ankylosing Spondylitis/Axial Spondyloarthritis established on treatment including biologic DMARDs

appointment within 365 calendar days. - No category 3 criteria
Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.
- History of symptoms (severity of back pain/stiffness and peripheral symptoms), evolution and rate of deterioration - Features of inflammatory back pain e.g. morning stiffness, young age, nocturnal pain, response to NSAIDs - Presence of psoriasis, inflammatory bowel disease, or uveitis - If on a b/ts DMARD please state the timeframe for renewal, and last renewed - FBC, E/LFTs, CRP and ESR - HLA-B27 (for Axial Spondyloarthritis ONLY) - Pelvic XR
