Cat 1 · 30 days
appointment within 30 calendar days. Patients who identify and require one of the following:
- Require assessment of chronic health conditions.
- Need support in creating or initiating a personalised care plan.
- Have a chronic condition, wish to access cultural support, and would benefit from group-based education.
Cat 2 · 90 days
appointment within 90 calendar days. - No category 2 criteria
Cat 3 · 365 days
appointment within 365 calendar days. - No category 3 criteria
Essential referral information
Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.
- Purpose of the referral – Cultural support, Chronic Disease education
- Confirmed chronic disease
- What does patient identify as: Aboriginal and/or Torres Strait Islander
Out of scope / wrong door
- Patient’s who do not identify as Aboriginal or Torres Strait Islander
- Patients under the age of <18 (Paediatrics)
- Maternity patients
- Patients that reside in a residential aged care facility (RACF)
- Patients that are medically deemed to not have capacity
- Mungulli Service does not offer respite or rehabilitation
