appointment within 365 calendar days. - No category 3 criteria

Essential referral information

Referral will be returned without this. If a mandatory test cannot be obtained, say so in the referral.

- Patient requires daily treatment - Diagnosis for CHITH treatment - Admitting team and responsible consultant - Medical history and history regarding inpatient admission - Reason for Physiotherapy intervention (if Allied Health referral) - Estimated date of transfer to CHITH - Length of treatment required on CHITH - Paediatric review and follow up plan documented - Completion of Referral Form [gchweb.sth.health.qld.gov.au/documents/CF000284]

Out of scope / wrong door

The following conditions are out of scope for this service: - Children over 18 years of age - Children who reside outside of the GCHHS catchment area or greater than 1 hours drive from GCUH - Children who are not medically stable or who are unable to receive cares in a community setting - Does not require daily clinical intervention
