Eligibility (published)

Screening (Yes/No): medically stable per GP guidelines; lacks clinically significant comorbidity (medical e.g. diabetes or severe MH diagnosis); help-seeking/willing to engage; does NOT have practical access to alternative provider (GP EDMP/private/self-fund); BMI 18.5 or above. If NO to any → ACT/AEDP pathway. Medical/psychiatric admission parameters in appendix.

  • Referral via Acute Care Team Intake 1300 744 284 (1300 MH CALL). ACT phone triage. GP Guide PDF on GCPHN. If NO to any screening question, call ACT; if YES to all, consider MHCP/EDMP in primary care instead. Fax observations to community team if already case-managed.
