Eligibility (published)

65+ or 55+ Aboriginal and/or Torres Strait Islander; lives at home not RACH not palliative; FRAIL scale 1 or 2 (score 3 may be accepted after BUnyAH review per PDF); at risk of frailty with ≥1 of: social isolation, poor self-care, recent hospitalisation, polypharmacy, multimorbidity, falls, poor nutrition, low socio-economic, mood changes; unable to access mainstream private/MBS services; Commonwealth pension and/or health care card; informed consent and can participate in groups; PDF also: good understanding of English without interpreter.
